The Homeless Outreach Program assists homeless and/or at-risk of being homeless individuals and families with healthcare and social support assistance by providing walk-in case management, and outreach services.

Case Management

HOP Case Management assists with health care and social support services by providing referral information and patient navigation. This Program works collectively with all FNCH’s services to provide comprehensive care and address health disparities experienced by our patients.

Job Development

FNCH Job Development offers FREE walk-in job training and computer lab services for homeless and/or at-risk of being homeless individuals. FNCH Job Development focuses on career planning and employment readiness.

Homeless Advisory Council (HAC)

The Homeless Advisory Council (HAC) meet the last Wednesday of every month at 1:00 pm at the All Nations Wellness Center to identify unmet needs and ensure FNCH services are culturally appropriate and meet their needs. Lunch is provided after the HAC meeting.

Healthy Living Education Group

The Healthy Living Education Group meets the last Wednesday of every month from 2:00 pm – 4:00 pm at the All Nations Wellness Center. Group is open to all community members and provides fun interactive physical activities for all ages, information on various health prevention/intervention topics, and information on upcoming community events.

Street Outreach

HOP Staff provide street outreach Wednesday afternoons from 1:00 pm – 4:00 pm and Friday mornings from 6:30 am – 10:00 am to promote FNCH services, provide health education material, and distribute food bags, hygiene products, and daily bus passes.
